LOHS Student Registration Form

Enrollment

Thank you for choosing Lake Oswego High School. Please contact the Counseling office at (503) 534-2314 to set up and appointment to register your student. We look forward to personally assisting you in this process. 

In order to complete your enrollment, you will need to provide LOHS with the following items.  

• Lake Oswego High School Student Registration Form

• Unofficial transcript from the previous school (all 10-12th graders)

• Immunization Records

• Signed OR Immunization Card (if not on file with your previous school)

• Proof of Residency (utility bill, rental/sales agreement)

• Custodial Documentation (if applicable)

• Copy of 504 or IEP documents (if applicable)
